Pharmacological properties:

Pharmacodynamics:
Sinestrol is a hormonal drug; its chemical structure differs from steroid estrogen hormones, but its biological and medicinal properties close to them.
The therapeutic effect of sinestrol is identical to the effect of follicular hormone preparations, both in terms of the effect on the uterus and mammary glands, and in terms of the effect on the cardiovascular, nervous system and on general condition female body. Under the influence of sinestrol, the blood supply to the genital organs increases and the processes of endometrial proliferation are activated. Sinestrol can cause engorgement and enlargement of the mammary glands due to the proliferation of glandular tissue.
The drug has the property of eliminating vegetative-vascular disorders that occur in women due to depletion of ovarian function (menopause, condition after castration).
Pharmacokinetics:
Absorption high, has a “first pass” effect through the liver.
Excretion kidneys; depending on the physiological state (individual phases of the menstrual cycle), age and other conditions, the rate of excretion is different.

Indications for use:

  • conditions associated with insufficient ovarian function (primary and secondary amenorrhea, oligomenorrhea, dysmenorrhea);
  • hypoplasia of the genital organs;
  • menopausal and post-castration disorders;
  • infertility; weakness of labor, post-term pregnancy;
  • prostate cancer, breast cancer;

Contraindications:

Hypersensitivity to the components of the drug.
Pregnancy; liver and kidney diseases; mastopathy, endometriosis, uterine bleeding of unknown etiology, as well as various forms of hyperestrogenism; diseases associated with increased blood clotting ability, malignant and benign neoplasms of the genital and other organs in women (under the age of 60 years).
Inflammatory diseases of the female genital organs (salpingoophoritis, endometritis), hyperfollicular stage of menopause, tendency to uterine bleeding.

Directions for use and dosage:

The drug is administered in the form of oil solutions intramuscularly.
  • For hypogenitalism, primary amenorrhea and underdevelopment of the uterus, use 1-2 mg intramuscularly daily for 4-6 weeks or more. If there is an effect, then progesterone or pregnin is prescribed for 6-8 days. With secondary amenorrhea, 1-2 mg of Sinestrol is prescribed for 15-20 days, then progesterone or pregnin is prescribed for 6-8 days at the indicated dose;
  • For oligomenorrhea - 1 mg intramuscularly every day or every other day during the first half of the intermenstrual period;
  • For infertility due to underdevelopment of the uterus - 1 mg intramuscularly in the first 7-8 days after menstruation;
  • For menopausal disorders, 0.5-1 mg is prescribed. The duration of treatment is individualized;
  • For prostate adenoma, 2 ml of a 2% solution of Sinestrol (0.04 g) is administered daily for 30 days. During the year, another 2-3 courses of 20 days are carried out with an interval of 2-3 months.
  • For prostate cancer, 3-4 ml of a 2% solution of Sinestrol (0.06-0.08 g) is administered intramuscularly daily for 2 months, then 0.5-1 ml of a 2% solution per day intramuscularly. The total dose and duration of treatment depend on changes in the prostate gland, the presence or absence of metastases, general condition and degree of feminization;
  • For breast cancer in women over 60 years of age, a 2% Sinestrol solution is used. Administer daily, starting with 1 ml per day, then the daily dose is gradually increased to 5 ml. Through careful observation, the optimal dose is determined and administered over a long period of time.
Higher doses of Sinestrol - for adults (intramuscular):
  • single dose - 0.002 g (2 ml of 0.1% solution)
  • daily - 0.003 g.
In the treatment of malignant neoplasms:
  • the highest single dose is 0.06 g (Zml 2% solution);
  • highest daily dose-0.1g (5ml 2% solution).

Side effect:

Nausea, vomiting, dizziness, rash, eyelid swelling, erythropenia.
With long-term use: in men - feminization (decreased sexual function, swelling and pigmentation of the nipples, reduction in the size of the testicles); in women - sclerosis of the ovaries, impaired sodium metabolism (peripheral edema), metrorrhagia.

special instructions
The drug should be prescribed only by the attending physician, under his supervision, after an accurate diagnosis has been established. Before use, the drug should be heated to 30-40°C. If crystals fall out, heat in a boiling water bath until they are completely dissolved.

Interaction with other drugs:

Antiestrogenic drugs (clomiphene citrate, etc.), used to treat infertility, can block estrogen receptors and weaken the effect of estrogens.
Folic acid and thyroid medications enhance the effect of sinestrol.
The side effects of sinestrol are reduced under the influence of progestins (gestagens), which serves as the basis for the joint prescription of sinestrol with gestagenic drugs (pregnin, progesterone, etc.). Sinestrol enhances the effect of lipid-lowering drugs, diuretics, antiarrhythmics, antihypertensive drugs, and indirect anticoagulants.

Overdose
When taking large doses, toxic liver damage is possible; excessive proliferation of the endometrium and uterine bleeding, peripheral edema, increased blood clotting. Severe feminization in men (decreased sexual function, swelling and pigmentation of the nipples, reduction in the size of the testicles).

Composition Sinestrol
The active ingredient is hexestrol.
Indications for use
In women, the indications for the use of folliculin are basically the same. Sometimes prescribed for hypertrophy (increase in volume) and prostate cancer in men.
pharmachologic effect
Sinestrol is a synthetic compound - a derivative of stilbene. Its chemical structure differs from steroid estrogen hormones (female sex hormones), but its biological and medicinal properties are close to them. Estrogens are female sex hormones produced by the ovaries and the adrenal cortex, causing the development of female genital organs, secondary sexual characteristics, and also promote the proliferation (growth) of endometrial cells (the inner layer of the uterus).
In terms of estrogenic activity (action similar to the action of female sex hormones - estrogens), Sinestrol is equivalent to folliculin: 1 mg of Sinestrol corresponds to 10,000 units.
Mode of application
Injected intramuscularly, subcutaneously and orally; intramuscularly and subcutaneously - in oil solutions, orally - in tablets. When administered orally, sinestrol is rapidly absorbed; is not destroyed in the gastrointestinal tract.
For hypogenitalism (underdevelopment of the external genitalia), congenital amenorrhea (absence of menstruation) and a severely underdeveloped uterus, use 0.001-0.002 g (1-2 mg) intramuscularly or 0.002 g (2 mg) orally daily for 4-6 weeks. and more. If there is an effect, then progesterone (5 mg per day intramuscularly) or pregnin (10 mg 3 times a day orally) is prescribed for 6-8 days. If necessary, courses of hormone therapy are repeated.
For secondary amenorrhea (cessation of normal menstrual cycles for a period of more than 6 months), 1-2 mg of sinestrol per day is prescribed for 15-20 days, then progesterone or pregnin at the indicated dose for 6-8 days.
For hypooligomenorrhea (scanty short-term menstruation - no more than 2 days), the drug is prescribed 1 mg intramuscularly or orally every day or every other day during the first half of the intermenstrual period; for infertility due to underdevelopment of the uterus - 1 mg intramuscularly or 1-2 mg orally in the first 7-8 days after menstruation; for menopausal disorders, 0.0005-0.001 g (0.5-1 mg) is prescribed orally. The duration of treatment is individualized.
For breast cancer in women over 60 years of age, a 2% solution of sinestrol is used. Administer daily, starting with 1 ml per day, then the daily dose is gradually increased to 5 ml. Through careful observation, the optimal dose is determined and administered over a long period of time.
For prostate cancer, 3-4 ml of a 2% solution (0.06-0.08 g) is administered intramuscularly daily for 2 months, then 0.5-1 ml of a 2% solution per day intramuscularly or orally. The total dose and duration of treatment depend on changes in the prostate gland, the presence or absence of metastases (spread of cancer to other organs and tissues), general condition and degree of feminization (appearance of female secondary sexual characteristics in men / growth of the mammary glands, fat deposition on the thighs, etc. .d./).
Sinestrol and other estrogenic drugs (see fosfestrol, folliculin) in the treatment of malignant neoplasms are usually used in combination with other methods ( surgery, radiation therapy).
Higher doses of sinestrol for adults orally: single - 0.002 g, daily - 0.004 g; into muscles: single - 0.002 g (2 ml of 0.1% solution), daily - 0.003% g. For the treatment of malignant neoplasms, the highest single intramuscular dose is 0.06 g (3 ml of 2% solution), the highest daily dose is 0. 1 g (5 ml of 2% solution).
Interaction with other drugs
No data.
Contraindications
Sinestrol and its analogues are contraindicated during pregnancy, liver and kidney diseases.
Side effect
Sinestrol is usually well tolerated. In some cases, nausea, vomiting, and dizziness occur. When taking large doses, toxic damage to the liver, excessive proliferation of the endometrium (increase in the volume and weight of the inner lining of the uterus) and bleeding in women, pronounced feminization in men (decreased sexual function, swelling of the mammary glands, pigmentation of the nipples, reduction in the size of the testicles, etc.) are possible.
